How to Do an ABCDE Assessment When Seeing an Acutely Unwell Patient

The approach to all deteriorating or critically ill patients is the same. The ABCDE approach is a systematic way of assessing a patient's condition and prioritizing treatment.

In reality, the A-E process is performed simultaneously rather than in a linear fashion; that is, establishing airway (A) and providing high-concentration oxygen (B) will occur at the same time as assessing breathing (C) and circulation (D), and decisions on management will be made throughout.

If the patient is conscious and able to talk, the history should be taken in parallel with the examination, using the OPQRST mnemonic. If the patient is unconscious, detail any preceding events and time of collapse. Then perform a primary survey.

Provide High-Concentration Oxygen Using a Mask with Oxygen Reservoir

Ensure that the oxygen flow is sufficient (usually 15 L/min^-1) to prevent collapse of the reservoir bag and maintain a positive mask pressure.

Conclusion

The ABCDE approach is a systematic way of assessing a patient's condition and prioritizing treatment. It is an essential skill for all healthcare professionals who care for acutely unwell patients.
